Consent
The purpose of this audit is to:
1. Identify the burden of COVID-19 in our community
2. Help to model rate of spread
3. Assist in access to formal PCR testing in the community
4. Assist in the prediction of healthcare needs in the hospital setting
5. Connect isolated community members with telehealth resources

Ultimately, the key to containing this virus is to keep people isolated who may have the virus while at the same time identifying those who need in-hospital with minimal exposure to those who may be vulnerable. The more quickly and efficiently we can do so, the faster we will be able to limit the spread and recover.
I understand that by using this website, the information I provide will be shared to the research team tracking COVID-19 in the greater Geelong Region and it's surrounds. All information will kept confidential and private as per the Barwon Health Information Health and Privacy Policies in place. I understand that I will be contacted to enable me to seek further advice on how to undergo formal testing for COVID-19 and get professional advice for treatment. The information will not be used for marketing or legal purposes. Any statistical data produced will be deidentified.
